This article was from 1901, but earlier, yes, close family marriages did occur. Why? There were several reasons. Families tended to move west together with their siblings’ families. They all belonged to the same church. They had large amounts of developed and undeveloped land between them and the neighbors.
Sunday church services were the most likely place to meet your future spouse. After asking permission, the young man would ask the young lady’s parents to ride with her by wagon (if he was of means) or by horseback. They would ride a respectable distance in front of the parents back to her house, with the parents close enough to discourage any hanky panky. After an early supper, he would leave to get back home before dark and ensure the young couple was not visiting without full light. So with all of the hanky panky prevention methods in place, there was not a lot of courting time for a young man to scour the countryside for a marriage worthy non-relative.

Likely not, but here are the reasons I have read over the years:
– It took a long time to get a proper exposure, so it was easier to “not move” with a frown than a smile.
– Smiling in a picture indicated a lack of decorum and suggested to others that you were a bit of an idiot.
If I had to choose, my most likely is that the posers often did not have such pretty teeth. There was no preventative dental care, orthodontia, and well water contained a lot of tooth-staining minerals.
